Tell a Friend and Earn a $50 Gift Certificate

To show our appreciation for sharing us with your family, friends, and colleagues, we will send you a $50 Gift Certificate for each person you refer to us who does a treatment. The $50 Referral Gift Certificate can be used towards any service, however cannot be combined with any other offer, promotion, or discount and products are not included. They may be combined with Allergan’s Brilliant Distinctions and Galderma’s Aspire Rewards Program.

When referring someone to EltraSelf, please ask them to give us your full name, as it will be the only way we can send you a gift certificate.

Due to the Patient Privacy Act (also known as HIPAA) under no circumstances can we confirm or deny that anyone has been to EltraSelf, regardless of your relation to the person, so we can only rely on what is recorded on a new patient’s paperwork.

Ensuring Your Timely and Quality Care

Please Review Our Cancellation Policy

You’re important to us and so is the treatment time reserved exclusively for you. Our goal is to provide you with quality care and treatment in a timely manner. No shows, last minute cancellations greatly impact our practice, doctors and service provider’s schedules. We strive to bring the most efficient service to our patients at all times. Please review our cancellation policy regarding cosmetic and spa appointments.

Late/Cancellation of an Appointment

If you are unable to make your scheduled appointment, we ask that you kindly call us within 24 hours to reschedule. Any patient with two consectuive cancellations, will be required to give a $50 deposit to schedule a future appointment.

*Failure to show up for that appointment will result in a $50 charge
or loss of their deposit.

If a patient is a NO SHOW for an appointment and would like to schedule another appointment, they will be required to give a $100 deposit to schedule their future appointment.

*Failure to show up for that appointment will result in a $100 charge
or loss of their $100 deposit.

*We understand that an emergency can happen and an exception may be considered.

No Returns or Exchanges After 2 Weeks from Date of Purchase - No Exceptions

Product can only be returned if there is a manufacturing defect or if the patient has a proven reaction to a product.
Any exchange in product resulting in a price difference will result in an office credit toward another product, service, or treatment only.

Consultations

Cosmetic Consults are $100 (applicable towards your treatment or service).

Medical Consults are typically $395 for non- insurance or out of network patients. There may be an additional fee should a patient need more comprehensive testing. When calling our office for an appointment, please provide us with your insurance information.
